By signing this document, you are giving up certain legal rights, including the right to sue. Participants must be
18 years of age or older, unless accompanied by a parent or legal guardian who has completed and signed the
minor consent section below.
1. Acknowledgment and Assumption of Risk
I, the undersigned, voluntarily choose to participate in the Mango Mustache Haunted House (the “Event”),
operated by Mango Mustache LLC (“Company”), and acknowledge and understand the following:
- The Event contains graphic and disturbing scenes, including blood, gore, loud noises, strobe lights, sudden
movements, depictions of extreme violence, and intense visual and audio effects.
- The Event may include props or artistic displays with slight nudity, which are part of the horror and thematic
design. No live actors appear nude.
- Live actors may scream, get close to, and lightly touch participants as part of the performance.
- The Event may cause fear, stress, dizziness, disorientation, or physical and emotional discomfort.
- The Event may include dark areas, uneven surfaces, narrow passageways, fog machines, and obstacles.
- Participants may be required to crouch, duck, or navigate through low-clearance spaces.
- I understand that participation involves inherent risks, including personal injury, psychological distress,
property damage, illness, or death.
2. Health and Safety
I certify that I am in good physical and mental health and do not have any condition (including but not limited to
heart problems, epilepsy, pregnancy, sensitivity to strobe lights, or claustrophobia) that would make my
participation unsafe. I agree to immediately exit the haunted house and notify event staff if I experience any
discomfort, distress, or injury.
3. Release of Liability
In consideration of being permitted to participate, I, for myself and my heirs, assigns, personal representatives,
and next of kin, hereby release, waive, and hold harmless Mango Mustache LLC, its owners, employees,
contractors, volunteers, affiliates, sponsors, vendors, and representatives from any and all liability, claims, or
causes of action arising from or related to any loss, injury, damage, or death during participation, whether
caused by negligence or otherwise.
4. Conduct and Rules
I agree to follow all posted and verbal instructions from Event staff. I will not touch, grab, or harm any actor, staff
member, or prop.
Any assault, aggressive contact, threats, or harassment toward actors, staff, or other guests—for any reason
whatsoever—will result in immediate removal and prosecution to the fullest extent of the law.
Any person who damages or destroys property, whether through negligence, recklessness, or intentional acts,
will be removed immediately and may be held financially and legally responsible for all damages and subject to
prosecution.
The Company reserves the right to remove any participant behaving in a disruptive, threatening, or unsafe
manner without refund or further notice.
5. Media Release
I grant permission to Mango Mustache LLC to use photos, video, or audio recordings taken during the Event for
promotional or marketing purposes, without compensation.
6. Governing Law
This agreement shall be governed by the laws of the State of Iowa. Any disputes shall be resolved in the courts
of Linn County, Iowa.
7. Acknowledgment of Understanding
I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY, FULLY UNDERSTAND ITS TERMS, AND SIGN
IT FREELY AND VOLUNTARILY. I UNDERSTAND THAT BY SIGNING THIS FORM, I AM GIVING UP
SUBSTANTIAL RIGHTS, INCLUDING THE RIGHT TO SUE.
